My apologies, but I am no longer accepting new insurance-pay patients. You may request an appointment if you agree to self-pay at the time of services. Documents for you to submit to your insurance can be provided upon request.

Appointments will only be available during school hours. I acknowledge that I expect to be seen within this schedule.

I agree to self-pay by providing a valid credit card to be kept on file. Card will be charged for services rendered, late cancellations and missed appointments as indicated in intake paperwork. I acknowledge that no insurance claims will be processed on my behalf by KB Therapy.
