My apologies, but I am no longer accepting new insurance-pay patients. You may request an appointment if you agree to self-pay at the time of services. Documents for you to submit to your insurance can be provided upon request.

The client portal is where you will receive required paperwork and you will be able to select an available appointment time. Please watch for an email from Your client portal invitation will expire after 72 hours. Please feel free to call 435.512.5100 if you have any trouble navigating the client portal.
