New Client Request

New Client Notice:

I accept new insurance-pay patients with the following providers: BCBS, PEHP & EMI. If you have insurance from a different provider, you may request an appointment if you agree to self-pay at the time of services. If self-paying at time of service, a "superbill" can then be provided to be submitted for reimbursement from your insurance. Reimbursement success varies by insurance provider and specific policy. If reimbursement options are important to you, this is something you will want to verify prior to submitting appointment requests.

Appointments will only be available during school hours. I acknowledge that I expect to be seen within this schedule.

I agree to self-pay by providing a valid credit card to be kept on file. Card will be charged for services rendered, late cancellations and missed appointments as indicated in intake paperwork. I acknowledge that no insurance claims will be processed on my behalf by KB Therapy if my insurance is not provided by BCBS, PEHP or EMI.
