When completing the new client request form (above), you will be given the option to request a coordination call or receive a link for your client portal. The client portal is where you will receive required paperwork and you will be able to select an available appointment time. Please watch for an email from TherapyPortal.com. Your client portal invitation will expire after 72 hours. Please feel free to call 435.512.5100 if you have any trouble navigating the client portal.
